By completing a Membership Shares Application Form to become a Member of, and investor in Ashendon Pub Community Society Limited, you are accepting the Share Offer Terms & Conditions for the value of shares specified and agreed on by the committee.

  1.  to buy shares for yourself, as an individual?  Scroll down to Form 1

  2.  to buy shares on behalf of a group or syndicate of up to 10 people?  Scroll down to Form 2

  3.  to buy shares on behalf of a business, organisation, or other incorporated or unincorporated   body? Scroll down to Form 3

  4. to buy shares as a gift for someone else over 18?  Scroll down to Form 4

  5. to buy shares on behalf of children under 18?  Scroll down to Form 5

INDIVIDUAL

Use this form if you are an adult individual buying shares in your own name.

GROUP/SYNDICATE

Use this form if you are an adult buying shares as a representative of a group or syndicate.

BUSINESS/ORGANISATION

Use this form if you are buying share as as a representative of a company or organisation.

AS A GIFT FOR ADULTS

Use this form if you are an adult individual buying shares for another adult as a gift.

ON BEHALF OF CHILDREN

Use this form if you are an adult buying shares on behalf on children under the age of 18.

Submitting Your Application Form

Option 1 - Digital Application Form
Once you have clicked the button, you may need to look in your Downloads folder to find this form. Complete the form digitally. Select 'Share' and 'pdf' and email it to ACPSLmembers@gmail.com 
(Please transfer funds to 'Ashendon Community Pub Society Limited' at the time of your application)
 

Option 2 - Manual Application Form 
Print out the pdf form. Complete it by hand.
a) Scan and email it to ACPSLmembers@gmail.com or
b) Post or hand deliver your printed Form to:
The Treasurer, ACPSL, Ashendon Farm, Ashendon, Bucks HP18 0HB.
(Please transfer funds (preferred method) at the time of your application or enclose a cheque with your form)
